Notice of Risk of Serious Injury

I understand that Group or Individual Personal Training, Self Defense training, fitness exercises, and all other activities that take place during a Patient Warrior Fitness session can be strenuous, and I may be subject to risk of serious injury. During my participation I may injure myself, I could injure others, or I may be injured by others. If I exercise, train, spar, or participate in any activities at a Patient Warrior Fitness session, I do so entirely at my own risk and agree to abide by the rules, conditions, and instructions set forth by the instructor.

Assumption of Risk

I understand that the risk of injury from participation at a Patient Warrior Fitness session is significant and includes, without limitation, the risk of (1) minor injuries such as scratches, bruises, and sprains, (2) major injuries such as joint and back injuries, broken bones, dislocated shoulders, heart attacks, and concussions, (3) catastrophic injuries including, without limitation, paralysis and possible death, and (4) being hit by or making contact with moving or stationary objects or people. I understand that while rules, equipment, and personal discipline may reduce these risks, the risk of personal injury at a Patient Warrior Fitness session, whether caused by me or caused by others, still exist, and cannot be eliminated regardless of the care taken to avoid injuries. I acknowledge that training, sparring, and or participating in any activity at a Patient Warrior Fitness session is dangerous, and I may be in danger of serious injury or possible death. Despite the potential hazards and dangers associated with participating in a Patient Warrior Fitness session, I wish to participate and voluntarily accept and expressly assume all risks, dangers, and hazards whatever they may be that could result in personal injury to myself or damage to my personal property.

No Medical Personnel or Equipment present

I understand that the instructor is not a medical professional, and does not have any medical equipment to treat any potential injuries that may occur. I understand that I am entirely responsible for the medical care and treatment of any injury that results from my training, sparring, or any activity that takes place during a Patient Warrior Fitness session. This includes being able to protect or defend myself, and administer or provide any required on the spot first aid if I get hurt.

Full Waiver of Everything Against Everyone Involved

By Signing this agreement, I am indicating that I have read and fully understand this agreement. By signing this agreement, I am waiving all claims of any nature whatsoever against the “Released Parties” listed below:

1.)    Patient Warrior Fitness LLC

2.)    The owners and/or tenants of the property where any Patient Warrior Fitness session is being conducted.

3.)    Antonio LeKeith Brown

I hereby am releasing, the released parties from all liability, that may arise in connection with my participation at a Patient Warrior Fitness session, including without limitation, any claims or liability based on negligence.

Waiver of Liability and Release of Claims

I do hereby release, remise, and discharge each of the Released Parties, and covenant not to sue any of the Released Parties for anything in connection with participation at a Patient Warrior Fitness session, and waive any liability of the Released Parties for any and all claims (including, without limitation, the negligence of any of the Released Parties resulting in personal injury) as a result of accidents, illness, or property loss, arising from or in connection with participation in a Patient Warrior Fitness session. I intend for this waiver and release to also apply to my relatives, personal representatives, heirs, beneficiaries, and next of kin, or assigns who might pursue any legal action or claim on my behalf. I understand that by signing this document I am giving up my right to sue any of the Released Parties.
